LA PLATA COUNTY PUBLIC HEALTH - Part-time Seasonal Clinic Nurse: Performs professional public health nursing services focusing on immunizations. This role will function in an outreach setting & transport vaccines to offsite locations; maintains health records. Requires BSN or ADN & PH experience preferred. Must possess Limited Practice Nurse cert or RN in the state of CO. Paid sick, vacation & holiday leave benefits. Hours include one 9-hour shift/week thru April 2025. JOB SUMMARY: Performs professional public health nursing services focusing on prevention including: family planning, child health, adult health, sexually transmitted disease, preparedness, communicable disease, breast and cervical cancer control program, refugee health, tuberculosis, immunizations, and primary care. Individuals in this role carry out health screenings within clinic or home environments. They educate clients about health matters and the corresponding treatments or medications, offering referrals when necessary. They assess and appraise the health requirements of clients, administer vaccines and tests, manage client cases, investigate instances of diseases, maintain accurate health records, and coordinate supplementary health support.

Inspections and Assessments Conduct screenings and assessments to identify health risks and potential health issues within the community. Plan and execute vaccination clinics and immunization programs to prevent the spread of communicable diseases. Monitor disease trends and outbreaks, and take appropriate measures to control and contain them. Consults with clients and administers immunizations in accordance to the vaccination schedule or as needed for travel. Manages supplies and vaccine stock. Ensures compliance with vaccine storage requirements and monitoring. Establishes and maintains relationships with community members, leaders, and organizations to better understand their health needs and concerns. Organize and participate in community health fairs, workshops, and events to provide health information and services. Provides individual and group counseling on various health topics, such as nutrition, family planning, smoking cessation, and substance abuse. Support individuals in making informed decisions about their health and lifestyle choices. Collects and analyzes health data to identify health trends, disparities, and emerging health concerns. Uses data to inform decision-making and tailor interventions to meet the specific needs of the community. Collaborates with other healthcare professionals, social workers, educators, and community leaders to coordinate and deliver comprehensive health service. Contribute to interdisciplinary teams focused on improving community health outcomes. Maintains accurate and up-to-date records of patient interactions, interventions, and outcomes. Prepares reports and documentation required for program evaluation, grant applications, and regulatory compliance. Overall, public health nurses play a pivotal role in improving the overall health and well-being of communities by addressing health disparities, providing health education, and delivering essential healthcare services. The specific duties and responsibilities of a public health nurse can vary based on the needs of the community and the organization they work for. Performs other duties as assigned. Regular and predictable attendance is required.

MINIMUM QUALIFICATIONS REQUIRED: Any combination of education and experience that would likely provide the required knowledge, skills and abilities is qualifying. A typical way to obtain the knowledge, skills and abilities for this position would be:
Education and Experience: Holds a Bachelor of Science in Nursing degree (BSN) with training in community and population health; or Associate Degree in Nursing (ADN) with minimum 5 years’ experience in a related area (e.g., a labor and delivery nurse applying for a role in a Maternal/Infant health programs).
Licenses and Certifications: License to practice as a Registered Nurse in the State of Colorado. Valid driver’s license Bilingual in English and Spanish/another language strongly preferred.
KNOWLEDGE, SKILLS, AND ABILITIES:
Knowledge of: Public health theory and practices. Nursing process. Nursing assessment skills. Basic human physical, nutritional and psychosocial needs. Federal, state, and county laws and regulations that impact the department. Electronic medical record documentation standards. Basic knowledge of statistics/data, community based participatory research.
Skill in: Maintaining records, reports and charts for completeness and accuracy. Coordinating deadlines, prioritizing work demands and assigning/monitoring work performed. Providing attention to detail in assignments. Basic budgeting. Administering injections. Interpreting and applying applicable laws, codes, regulations and standards (based on assignment).
